Pelvic Floor Physical Therapy

Pelvic floor physical therapy is a targeted approach designed to help women manage and treat various pelvic floor disorders, including pelvic floor dysfunction. At our clinic, we have an in-house female pelvic floor physical therapist, who specializes in the anatomy and function of female pelvic floor muscles. The treatment typically spans 8-10 weeks and involves personalized exercises and techniques to strengthen and relax the pelvic muscles, effectively alleviating symptoms. This therapy is highly effective for those who commit to the full course; however, it requires a consistent commitment to therapy sessions. Our collaborative approach ensures that your treatment is coordinated closely with your overall care.

Bulkamid® Injection

Bulkamid injection is an option to resolve stress urinary incontinence. The procedure consists of four injections of a smooth, water-based gel within the wall of the urethra (the tube that allows urine to leave the bladder). This quick, outpatient procedure typically takes only about 5-10 minutes and can provide immediate improvement with minimal discomfort and no need for recovery time. Bulkamid offers a safe and effective option for women seeking a long-term solution to improve their quality of life without undergoing more invasive surgery.

Interstim®

InterStim, a form of sacral neuromodulation, is a therapeutic device used to treat overactive bladder (OAB). It involves the implantation of a small device that sends electrical impulses to the sacral nerves, which control the bladder and muscles related to urinary function. This neuromodulation helps to normalize communication between the brain and bladder, reducing symptoms of urinary urgency, frequency, and incontinence. It is also an effective treatment for fecal incontinence after more conservative treatments have failed. 

Bladder Botox®

Bladder Botox, or onabotulinumtoxinA injection, is a minimally invasive treatment for overactive bladder (OAB) when other therapies haven't provided relief. During the procedure, Botox is injected into the bladder muscle, temporarily paralyzing it and reducing involuntary contractions that cause urgency and leakage. This helps to decrease urinary frequency and urgency, providing significant relief for individuals with OAB symptoms. The effects typically last several months before requiring re-treatment.

SURGICAL OPTIONS


Robotic Surgery

Robotic surgery has revolutionized the approach to pelvic organ prolapse. We can now perform these surgeries on an outpatient basis, often allowing patients to return home the same day. Recovery times are dramatically reduced, with many patients able to resume work and normal activities within just a couple of weeks. Our team, the highest volume of robotic female pelvic floor surgeons in New Jersey, leverages this advanced technology to ensure optimal outcomes with minimal disruption to our patients' lives.

Robotic Sacrocolpopexy

Robotic sacrocolpopexy is a minimally invasive surgical procedure used to correct pelvic organ prolapse, where organs like the bladder, rectum, or uterus descend into the vaginal canal. Utilizing a robotic-assisted approach, the surgeon attaches a synthetic mesh to the prolapsed organ and secures it to the sacrum (the lower backbone), providing support and restoring normal anatomy. This technique offers greater precision, smaller incisions, reduced pain, and faster recovery compared to traditional open surgery.  Most patients experience minimal downtime and return to their daily activities within 2-4 weeks after the surgery. Robotic sacrocolpopexy is the most durable prolapse repair with high success rates and low likelihood of recurrence. 

Mini Sling

The mini sling is a contemporary, less invasive treatment option for women experiencing stress urinary incontinence. It involves the placement of a small, mesh sling under the urethra to provide support and prevent urine leakage during activities that may cause leakage such as coughing or exercise. The procedure is quick, typically performed under twilight anesthesia, and allows for a rapid recovery. Importantly, the mesh used in mini slings is FDA-approved and has been rigorously tested for safety and effectiveness, offering a reliable and long-term solution for managing symptoms of incontinence.

Cystocele Repair

A cystocele repair is a surgery to fix a condition where the bladder descends into the vagina, causing discomfort and urinary issues. During this outpatient, vaginal procedure, the surgeon lifts the bladder back into place and reinforces the wall between the bladder and vagina with stiches. Recovery typically involves a few weeks of avoiding heavy lifting and strenuous activities to allow healing.  There is typically minimal discomfort afterward. Most patients can return to normal activities gradually, with significant relief from their symptoms.

Rectocele Repair

A rectocele repair is a surgery to fix a condition where the rectum bulges into the vagina, causing discomfort and bowel issues. During this outpatient, vaginal procedure, the surgeon pushes the rectum back into its proper position and reinforces the vaginal wall with stitches. Recovery usually involves a few weeks of avoiding heavy lifting and strenuous activities to ensure proper healing. Most patients can gradually return to their normal routines, experiencing relief from their symptoms and improved bowel function.

Cosmetic Labiaplasty

A cosmetic labiaplasty is a surgical procedure to reshape or reduce the size of the labia minora, the inner folds of skin surrounding the vaginal opening. This surgery is often performed for aesthetic reasons or to relieve discomfort caused by elongated or asymmetrical labia. The procedure involves trimming excess tissue and contouring the labia for a more desired appearance. Recovery typically includes a few weeks of avoiding strenuous activities and following specific care instructions to ensure proper healing.

Vesicovaginal Fistula Repair

Vesicovaginal fistula repair is a surgical procedure to correct an abnormal opening between the bladder and the vagina, which can cause continuous urine leakage. Depending on the fistula's location, the repair can be performed either vaginally or using robotic-assisted surgery. The surgeon closes the fistula to restore normal urinary function and prevent leakage. Recovery involves following specific care instructions and avoiding strenuous activities for several weeks to ensure proper healing.

A urogynecologist is a specialist who focuses on the diagnosis and treatment of pelvic floor disorders in women, such as urinary incontinence and pelvic organ prolapse.   While most urogynecologists complete medical school followed by specialized training in obstetrics and gynecology, our doctors at NJ Urogyn pursued a distinct path. They completed extensive training as board certified urologists followed by a pelvic floor reconstructive surgery fellowship. This dual-expertise equips them with a comprehensive understanding of the urinary system and pelvic floor disorders, enabling them to provide exceptional care for these conditions.

Pelvic floor disorders are conditions that can cause physical discomfort and affect your quality of life, often interrupting everyday activities like work, exercise, travel, and intimate relations.The most common conditions include urinary incontinence, pelvic organ prolapse, fecal incontinence, recurrent urinary tract infections and pelvic pain.

There are many things that can lead to a pelvic floor disorder. The most common factors include childbirth, family history, aging, smoking, obesity, repeated heavy lifting, previous surgeries, and straining for bowel movements. 

The most common symptoms include involuntary leakage of urine or stool, a feeling of pressure, fullness, or pain in the pelvic area, and/or pain during sex. Some other signs include vaginal bulging, trouble urinating or emptying the bladder, frequent or urgent need to urinate, and/or frequent urinary tract infections. While these symptoms can be embarrassing to talk about, they can lead to more serious health problems when left untreated.
